**Opposition from community health clinics and medical associations has driven trader expectations that California Proposition 44 will fail.** The measure, backed by SEIU-UHW, would require federally qualified health centers to spend at least 90% of revenue on program services advancing their charitable mission, with penalties for shortfalls and added reporting to the Attorney General. Clinics report current averages near 80%, and independent analyses project roughly $1.7 billion in first-year penalties plus risks of service reductions or closures for most affected providers. The California Primary Care Association and groups including the California Medical Association have mounted legal challenges and funded a coordinated “No” campaign that has raised over $33 million—nearly double the support side—highlighting institutional resistance ahead of the November 3, 2026 vote. This funding and organizational edge, rather than polling data, underpins the current 68% implied probability of rejection.
